CCHA Services (Medicaid RAE 6 & 7)

Colorado Community Health Alliance (CCHA) is the Health First Colorado (Colorado’s Medicaid Program) regional organization. CCHA administers physical and behavioral health benefits and coordinate care and services for approximately 380,000 members.

CCHA’s goal is to help you get the most from your health care and live a healthy life. CCHA care coordinators work together with you, your primary care provider, and your family members/caregivers. As part of your health team, CCHA care coordinators learn about your needs and help create a plan to get you on track.

  • Colorado Community Health Alliance (CCHA) is your regional organization if you receive primary care one of these counties: Boulder, Broomfield, Clear Creek, El Paso, Gilpin, Jefferson, Park or Teller. CCHA works with providers in these counties to make sure you get the care you need.

  • CCHA services are part of your Health First Colorado benefits, so there is no cost to you.

How do Care Coordinators Help?

CCHA care coordinators can help you navigate the health care system and connect you with medical and non-medical services. They can help you:

  • Understand Health First Colorado benefits

  • Find a primary care doctor, behavioral health provider or specialist

  • Coordinate physical and behavioral health, dental, vision, home health, and long-term care needs

  • Connect with community resources

  • Enroll in programs to help manage chronic conditions, like diabetes

With CCHA as your regional organization, you have a whole team that works together to guide you through the health care system. We also help connect you to the right services to meet your unique needs. These services may include physical health, behavioral health, dental care, and other resources such as food, housing, transportation and childcare.
