RHMPs Services (Medicaid Western Slope RAE)

RMHP can provide case management, care planning, and referrals to local community resources for members who qualify. The member will be placed with a case manager, free of charge, who will provide an assessment, service planning, and follow-up for those receiving long-term care. More Info:

  • SEP services are offered to members of Health First Colorado (Colorado’s Medicaid Program). Members who receive care are typically adults over the age of 18, but children in need of long-term care may also benefit from SEP services.

    To receive these services and to have the cost of care paid by Health First Colorado, Members must:

    qualify financially for Health First Colorado, which is determined by a county eligibility specialist

    have an assessment of their functional capabilities

  • At RMHP, we’re here to help our members get the quality care that’s right for them. To do this, we work with our members, providers, and a variety of community resource programs. RMHP is the Single Entry Point (SEP) agency in Mesa County, Costilla County, Conejos County, Mineral County and Rio Grande County.

    Our goal as the SEP is to help our members find the care that’s right for their individual needs while helping them maintain their independence as much as possible.

    As the SEP, we will determine if a member is eligible for long-term care services, like home- and community-based services, long-term home health, nursing home care, and home care allowance. We will then serve as the point of contact between programs and members in need of this care.
